Biological age estimates how old your body actually functions, separate from your chronological age, the number of years since you were born. It is calculated from biomarkers such as blood work, muscle strength, and cardiorespiratory fitness rather than a birth certificate. Two people the same chronological age can have biological ages years apart depending on genetics, disease burden, and daily habits.
The Mechanism
Scientists estimate biological age using several different biomarker panels, and the panels often disagree with each other. The most common approach analyzes DNA methylation patterns, chemical tags on your genes that shift in predictable ways as cells age; this method, often called an epigenetic clock, is one specific technique for estimating biological age, not a synonym for the concept itself. Other calculators build a phenotypic age score from routine blood work, combining markers like glucose, liver enzymes, and inflammation with your chronological age into a single composite number.
A separate family of estimates skips lab work and leans on physical performance instead: aerobic fitness (VO2 max), grip strength, and walking speed all correlate with how well organs and tissues are holding up, and some calculators use these measures alongside or instead of blood biomarkers. Because each method measures a different biological signal, the same person can get a noticeably different number from a methylation clock than from a fitness based estimate. That difference is expected. Biological age is a directional signal about how your body is aging relative to your peers, not a diagnostic result with one correct answer.

Biological age gives you a way to track whether your daily choices are compounding in your favor, years before a diagnosis would show up on a chart. It reframes health decisions around a measurable trend instead of a fixed birthday, which matters most when two people of the same chronological age face very different disease risks. No biological age test is validated yet as a clinical diagnostic tool, so it is best treated as a motivational trend line rather than a medical verdict.

People often use "biological age" and "epigenetic age" interchangeably, but epigenetic age refers specifically to DNA methylation clocks. It is one method among several for estimating biological age, and different methods can disagree by years for the same person because they measure different aging signals.
